I am trying to configure a form where one field is duplicated at several places on the form. But, I want the field to be filled in only once at the top of the form but the data shows in all the duplicate fields.

The result is that the other fields would show the data ( a name) but not be included in the tab order or be modifiable.

Is that possilble?

I need to give my radio buttons a value that can be calculated to create a rating. For example, a list of questions is provided and each question can be ranked 1=poor, 2=average, 3=above average, 4=very good, 5=excellent. I want radio buttons so only 1 of the options can be selected at a time. I then want to calculate all of the ratings and divide this total by the number of questions to get an average rating.

I can't find any way to create radio buttons that export a value if selected.

THANK YOU SOOOOOOOOOOOOO MUCH. I was having the worst time trying to create radio buttons per Adobe's instructions. Your screenshots are awesome. :-)

Do I have to re-create my list in the combo box every time I need it? Can you save a list to be used again in a different form or the same form?

This definitely needs a follow up to see how this form delivers information back to the company that generated it.

Obviously it can just be read and the info is processed by a human being, but how can it be automated or used to collect massive amounts of data more automatically?

OK. So I have the form. Can you help me with what happens when the form is submitted? How is the info calculated and stored? Where does the form come back -- email address? Good info so far. Thanks.
